Moving Forward Project - Tenancy Support
The aim of the project is to offer floating or visiting support
to people with vision impairments across the city in any tenure
to enable them to resettle and / or sustain their accommodation.
The service components are to:
- Undertake assessment of support needs and agree support plans
- Deliver the service to service users via visits to them at
home and/or other suitable venues/locations
- Monitor the progress of service users against support plans
and regularly review those plans
- Undertake signposting and referrals to other appropriate agencies
and services
Key service outcomes will be to enable clients to:
- Gain necessary adaptations to their accommodation
- Manage and retain their accommodation
- Access welfare benefits
- Access healthcare
- Respond to communication received, including welfare benefits
